This waiver is valid for 364 days from the date of signing.

To ensure that your surf session on the Citywave will be an unforgettable experience, we kindly ask you to read the terms and conditions carefully and sign below.

I am aware that surfing on the Citywave is a physical activity and that my participation involves a risk of injury. I wish to surf and participate in the booked sessions at my own risk.

Non-swimmers are not permitted to use the facility!

For participants under the age of 18, a parent or legal guardian must sign this form and be present at all times during the use of the facility.

YES, I MEET THE FOLLOWING participation requirements of the Citywave:

My weight does NOT exceed 120 kilograms.
I have NO shoulder or joint problems.
I have NO back, disc, or spinal problems.
I have NO cardiovascular disease.
I do NOT have a pacemaker.
I have NOT suffered a stroke.
I am NOT under the influence of alcohol or drugs.
I currently have NO acute injury.
I am NOT suffering from any acute illness.
I have NO disease of the brain, balance organ, or nervous system.
There are NO other physical conditions that Citywave (AWSM GmbH) should be informed of prior to my surf session.
I am OLDER than eight years.
For female surfers: I am NOT pregnant.

I am aware that by using the facility, I automatically give my consent to the free recording of photos, live images, and videos.

I am familiar with the full terms and conditions and confirm the above statements.
